GENERAL FORMS

Option Enrollment applications for the 2027-28 school year will be accepted beginning September 1, 2026.
*Applications submitted or dated prior to September 1 will be returned to sender.*

Students who do not reside within the boundaries of the Northwest School District are welcome to apply to attend Northwest schools as a non-resident student on a space-available basis in accordance with Nebraska law. Download an option enrollment application and addendum to option enrollment application for the 2027-28 school year. Submit the completed forms to Chelsie Van Buskirk at the District Office or email the forms to cvanbuskirk@ginorthwest.org. An enrollment packet, birth certificate, and immunization records will be required from new students upon acceptance into the Northwest School District.
